Please fill out this entire application and indicate in the "comments" section, that you are applying to be a volunteer.

ADOPTION REQUIREMENTS

● You must be 21 years of age and living on your own.

● All current pets in the household MUST be spayed/neutered.  The only exception is if they are currently being shown in the conformation ring or health issues preclude this surgery.  Verification will be required.

● All current pets MUST be up to date on vaccinations and be maintained on monthly heartworm preventative.  (Yes, we will call your vet to verify this information)

● We do not adopt out dogs as gifts or surprises! Absolutely no exceptions.

● If you rent, you must provide your landlord's name, address and phone number

 

*PLEASE NOTE*

FBR will not place two female dogs in the same home due to dominance issues often seen in such circumstances.  Based on our experiences, FBR feels we can increase successful adoptions by not placing females together in the same home.

 

Please do not fill out an application if you live in a state other than Florida; we do not "ship" dogs to you.
